New Student Form & Agreement
If you are a brand new, incoming student for in-person, individual lessons, you will need to complete this entire packet including the check list. There is a ton of studio information in this packet, so if you are looking to sign-up for individual lessons and either wanting more info OR you are already on the waitlist and want to be prepared, this packet can give you all the ins and outs of studio policies, (cancellations & makeup lessons, stopping lessons, in-class expectations, ECT...) materials needed, any fees or additional costs that may come up, available discounts, and liability agreements.
Scholarship Application
If you are a low-income family and want to apply for your child, this is the form you will want. Please naviage to the 'Programs' tab, then 'Scholarship' for more information before applying.
